lily's world XD Incident Report
The Incident Report is the last puzzle in the game and the reason for most of the earlier paperwork. It asks for two IDs, both of which are built from names you have already read.
Quick answer
Open theradiantpath.com, sign in to the Members Area, pass the Commune check with 43521, and continue to the Ohio Site, where the Incident Report waits. The two fields take CA-1947 for the commune manager and LH-2834 for Lily. After you read the report, Share Files triggers the ending.

The path to the report
Four screens stand between the Members Area login and the report. Two of them are also where the required IDs are written down.
- Sign in to the Radiant Path Members AreaThe address and the shortcut credentials come from the letter Iris sends near the end. Without that login the Members Area form will not accept anything you try.
- Pass the Commune knowledge checkThe Commune asks for a five-digit code built from the group's own history. All sources converge on
43521. Do not bother reconciling the founding month, because sources disagree on it and it does not change the code. - Move on to the Ohio SiteThe Ohio Site is the section after the Commune. It holds the Retreat Schedule and member records, which is where the manager's identifier is established.
- Open the Incident ReportThe report is the last item inside the site. It opens a form with two fields and nothing else, so there is no way to brute-force past it by exploring.
The two IDs and the rule behind them
Both identifiers follow one pattern: the person's initials, a dash, and a four-digit number. Once you know the pattern, the second ID is mostly a matter of finding the name.

The initials come from the name the game uses for each person, and the four digits are a staff or subject number that appears next to that name. Reading the Retreat Schedule and the forum thread is therefore enough to build both entries without guessing.
What the report contains
The report is the payoff for the whole investigation, and it is short. Two pages and a voice note carry the ending's weight.
Inside are several images, including two report pages, and an audio voice note. The pages describe what happened to Lily at the end of her time with the group, and the voice note records a discussion about her medication being increased.
One walkthrough describes the sequence as a forced cosmetic procedure, an attempt to leave, and a fatal complication. That account has a single source behind it, so treat the specific cause as reported rather than confirmed, and let the pages speak for themselves when you read them.
If the report will not open
Almost every report on this puzzle comes down to one of three causes.
- A missing Commune code. The Ohio Site stays closed until the five-digit check is passed, and the field does not hint at its own answer.
- A field-name mismatch. The form's labels differ between transcriptions, so read the label on screen and match it to the table above rather than to a guide's wording.
- A hyphen or capital-letter difference. Enter the hyphenated upper-case form first, then the plain form.
The report cannot be reached before the computer resets, so if the Ohio Site does not exist in your browser yet, you are earlier in the story than this page assumes. The full walkthrough shows what has to happen first.
